4. Appointments and Cancellations

When booking an appointment, you agree to provide accurate and complete information. We require adequate notice for appointment cancellations. Failure to provide sufficient notice or missing an appointment may result in cancellation fees as outlined in our clinic policies provided to you during intake.

5. System Rentals

If you rent a NeurOptimal® system from us, you agree to:

• Use the equipment only as instructed and for its intended purpose.
• Assume responsibility for the care and safety of the equipment while in your possession.
• Return the equipment on the agreed-upon date in the same condition it was received.
• Be liable for any damages, loss, or theft of the equipment during the rental period.

6. SMS and Text Messaging

By opting in to receive SMS or text messages from DRW NeuroPerformance, you agree to receive communications regarding your inquiries, appointments, and care. You acknowledge that standard message and data rates may apply. You may opt out at any time by replying "STOP" to any message. We will not share your mobile information with third parties for marketing purposes.
